Participation agreement

Personal information

• participant´s per­sonal information (name, surname, address (just the town), age, phone number and email address)
• an agreement with using your personal data in relation of comparative study
• a declaration Optik Krouman^® declaration, that all personal information will be used only to a purpose of realisation of a study; the study will be published without participant´s per­sonal information

Participant´s de­claration

• you have no previous experience with wearing contact lenses
• you are familiar that you will get two different contact lenses, each one for one eye

Participant´s com­mitment to fulfill the requirement of a study realisation

• take part to first application and personal control in 14 days after the first application (the appointment will be discussed before, to your likelyhood)
• to fill in both questionnaires (on time, truly and fully)
• to leave the study just in case of serious personal or health inconvenience
• to pay a fee of 580 Kč / 24 € (later you can use 444 Kč / 18,50 € as a discount for your first purchase of contact lenses)

Optik Krouman^® declaration during the study realisation

• to provide professional technical and personal background
• to provide all accessories needed
• to offer 444 Kč / 18,50 € discount for your first purchase of contact lenses of our choice (you can use this discount 3–4 weeks after the day of first application)

Short study schedule

• 1st day to pass the first application including the following control and training in contact lenses handling, closely about the first application (it takes 60 minutes)
• approx. 7th day to fill in the questionnaire via internet, you will be alerted by SMS and/or email (it takes 5 minutes)
• approx. 12th day personal control by opticians (the appointment will be discussed before), you will get lenses for next 2 weeks, you will fill in the questionnaire with the optician (it takes 30 minutes)
• approx. 20th day to fill in the questionnaire via internet, you will be alerted by SMS and/or email (it takes 5 minutes)
• 3–4 weeks after the first application you can use the choosen contact lenses with the 444 Kč / 18,50 € discount

In case we find out astigmatism on your eyes, it is possible that the control and training in contact lenses handling will be held on other appointment (there is a possibility that we have to order individually your contact lenses)

Subsidiary rules

• if we find out some contraindication of contact lenses wearing (e.g. serious eye injury) and you are not able to be implied into the study, you pay no fee
• if you are not able to get over the study because of some discomfort (e.g. allergy), you can apply your 444 Kč / 18,50 € discount for buying new eyeglasses or frames
• of you pass the study and you will realize that contact lenses are nothing to you, you can also apply your 444 Kč / 18,50 € discount for buying new eyeglasses or frames
